I can help individuals create living well end-of-life/aging plans, as well as the necessary documentation to ensure that a patient’s final wishes are honored. This includes a personal plan for advance directives, living plans, and family mediation. Help prepare an end-of-life living plan: ➢   Explain what advance directives are:    ◆   Living will    ◆   Health care proxy    ◆   Designation of a conservator    ◆   Organ donation    ◆   Various medical terms ➢   Complete advance care planning documentation:    ◆   What treatments you want    ◆   What you don’t want    ◆   Facilitate family meetings Discuss your wishes with your family and health care provider.
